BASIC FUNCTION

As a member of the social services team, provide direct task-oriented social work services to clients/patients, families, and others; and/or participate in the practicum instruction of social work assistant students.

DISTINGUISHING CHARACTERISTICS

Under general supervision, provide direct task-oriented social work services and/or participate in the practicum instruction of social work assistant students.

TYPICAL WORK

Assist clients/patients, families, and others with discharge planning; coordinate with other staff members; explain program requirements (such as Medicaid, Medicare, Social Security Disability) to potential participants and referrers;

Participate in practicum instruction of social work assistant students;

Teach clients/patients, families, and staff task-focused aspects of discharge planning;

Perform the work of Social Work Assistant I’s;

Perform related duties as required.

MINIMUM QUALIFICATIONS

Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.

OR

an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
